Tourist Visa

  1. NOTARIZED Visa Application Form (Must be Physically Signed, Electronic Signatures are not accepted)
  2. Original Passport (Must be valid for at least 6 months and must have 2 blank visa pages)
  3. Proof of Status (Copy of your Green Card, F1 Visa, H1B Visa etc)
  4. 2 Passport Sized Photos (Please sign across the bottom of the Front of your photo)
  5. Flight Itinerary
  6. Hotel Reservation
  7. Latest Monthly Bank Statement
