Requirements for a child U.S. Passport
Step 1: Collect and put together the documents for a new U.S. Passport
- Fill the DS-11 Application Online
- Once done filling out the application, click on the link that says, ‘Create Form’ and a file will be downloaded to your computer.
- Do not sign this application form until you are instructed to, during the application sealing process.
- Gather Evidence of Citizenship: At least one of the following must be provided
- Birth Certificate, issued by state, county, or city.
- Certificate of Citizenship or FS-240, DS-1350
- Certificate of Naturalization
- Parental Authority: (Important – If the child is under the age of 16)
- Both parents must sign the application for passport
- If only one parent can be present, you must have a signed, NOTARIZED parental consent (DS-3053) and the non-present parent must submit a clear copy of their drivers license (both sides of the card)
- Only one parent can apply if that parent can provide evidence that they have sole custody to apply for the child or if only 1 parent’s name is on the birth certificate.
- Evidence of Credentials from both parent (Either one)
- Valid Driving License with photograph, issued more than 6 months ago.
- Valid Original Passport (Must be signed)
- Evidence of Urgent Trip
- A travel itinerary on applicant’s name (Must be within 14 days of travel)
- Passport Photographs
- Provide 2 Passport Size Photographs, front view and on a white background taken by a professional
- US Government Fees
- Cheque or Money Order payable to “U.S. Department of State for $140 (Government processing fees for a New Passport)
- Acceptance Agent Fees: Make the second check or money order in the amount of $35 payable to the post office or court house which you visit
- Authorization Letter : (Authorizes us to take steps as your agent)
- Print and complete 2 copies of the Expedited Passport Authorization Letter
- BOTH parents MUST sign this document unless ONE parent has sole custody of the child.
Once you are ready with the following documents please take it to an Acceptance Agent / Post Office (Enter your zip code and find out the closest location where your documents will get sealed)
Step 2: Authorization Processing
- Bring all the required documents before appearing in front of an acceptance agent
- This is what you will need to take:
- Completed DS-11 Application Form
- Evidence of Citizenship
- Evidence of Individuality
- Evidence of Urgent Tour
- Authorization Letter
- Passport Photographs
- The $140 Check or Money Order to the ‘U.S. Department of State’
Step 3: Review with us
- Once you have got your application sealed, please give us a call so we can go over a few important things with you.

- Government Fees should be in the form of Money Orders or Cheque
- Service Fees can be in any form including checks, money orders, credit cards / debit cards (Order Form)
