Sierra Leone Visa Requirements

1. Visa Application Form: To place an online order, please download, print, and sign the Sierra Leone visa application generated by our system using your account.

2. Original Passport: Your passport should have a validity of at least 6 months beyond the intended departure date from the destination country, and it must contain a minimum of 2 empty visa pages.

3. Passport-Style Photograph: Please include a single passport-style photograph with a white background, taken within the last 6 months.

4. Yellow Fever: You are required to provide a copy of the International Certificate of Vaccination (ICV) for yellow fever. For more information about the yellow fever vaccination or to locate a vaccination clinic, please refer to the CDC's yellow fever resource page.

Additional Documents for Minors (Under 18 years):

• Birth Certificate: A copy of the applicant's birth certificate is necessary.
• Notarized Application Form: Please complete the paper application form and ensure that it is signed by both parents. The signatures must be notarized.
• Parents' IDs: Copies of government-issued identification cards from BOTH parents are required.
• Letter of Consent: A NOTARIZED letter of consent, signed by both parents, is necessary. The letter should specify the accompanying adult(s) during the child's travel and authorize the Consulate to issue a visa to the child.
