  2. Original Passport (Must be valid for at least 6 months and must have 2 blank visa pages)
  3. Proof of Status (Copy of your Green Card, F1 Visa, H1B Visa etc)
  4. Two Passport Sized Photos
  5. Yellow Fever Vaccination Copy
  6. Flight Itinerary
  7. Hotel Reservation
  8. Letter of Invitation from Host in Guinea
  9. Guinea Visa Application Form
