Visa Application Form (Must be Physically Signed, Electronic Signatures are not accepted¬)
Original Passport (Must be valid for at least 6 months and must have 2 blank visa pages)
Proof of Status (Copy of your Green Card, F1 Visa, H1B Visa etc)
2 Passport Sized Photos

Latest 3 Months Bank Statements
Proof of Residence (Driver’s License, State ID, or Utility Bill showing current address)
