  2. Original Passport (Must be valid for at least 6 months and must have 2 blank visa pages)
  3. Proof of Status (Copy of your Green Card, F1 Visa, H1B Visa etc)
  4. Biometrics (Original Stamped Sheet, you will receive this upon filling out your application)
  5. Itinerary
  6. Latest 3 Months Bank Statements
  7. Latest Letter of Employment
  8. Latest 6 Pay Stubs
  9. Latest Tax Returns (Either 1040 or Summary Pages)

Business Visa
